Annual Medicare Cost Estimator

On Original Medicare plus Medigap, your year is largely predictable: the Part B premium ($202.90/mo standard in 2026), your Medigap and Part D premiums, the $283 Part B deductible, and drug costs capped at $2,100 out of pocket (CMS.gov, Medicare.gov). Add your own quotes below.

Assumes premium-free Part A (40+ quarters — check with the Part A checker) and the standard Part B premium. Nothing you enter is stored.

Frequently asked questions

How much does Medicare cost per year?

On Original Medicare plus a Medigap plan, the predictable pieces for 2026 are $2,434.80 in Part B premiums (12 × $202.90), the $283 Part B deductible, and your Medigap and Part D premiums. Drug spending is capped at $2,100 out of pocket for the year (CMS.gov). The estimator adds your own quotes to those figures.

Why is Original Medicare plus Medigap so much more predictable than Medicare Advantage?

Because most of the cost is premium rather than cost-sharing at the point of care. Medicare Advantage typically has a lower or $0 premium and a much higher maximum out-of-pocket, so a bad health year costs far more. Neither is universally better — the right answer depends on your doctors, drugs and tolerance for variability.

What is the $2,100 cap?

Starting with the Part D redesign, once your out-of-pocket spending on covered drugs reaches $2,100 in a calendar year, covered drugs cost you $0 for the rest of the year (CMS.gov). The estimator caps your drug input there automatically.

Does this include IRMAA?

Only if you enter it. If your income triggers a surcharge, run the IRMAA calculator first and put the monthly figure in the IRMAA field.

Method: 2026 standard Part B premium $202.90/month and Part B deductible $283 (CMS.gov); Part D out-of-pocket spending capped at $2,100/year under the Part D redesign (CMS.gov). Figures change every plan year. This estimates cost, not coverage — model it with real carrier quotes and your drug list.
