Three Risks Sit Inside Your Plan and Outside Your Licence.

Nobody builds a retirement plan intending to leave healthcare, claiming timing and extended care unmodelled. It happens because each one needs a different licence, a different certification, and a different annual re-check.

The gap, specifically

A well-built plan projects income, sequences withdrawals, models longevity and stress-tests the portfolio. Then three things arrive that it usually has not priced:

1. Medicare is a permanent decision made in a seven-month window

The choice between Medicare Advantage and a Medicare Supplement is close to one-way. Take a Supplement at 65 and a client can move to Advantage later; take Advantage at 65 and moving back may require passing medical underwriting. That is not a detail — it is a decision that changes a household's healthcare costs and provider access for thirty years, and it is usually made in the weeks around a birthday with no analysis attached. We wrote up the mechanics in what happens if you miss your Medigap window.

2. Claiming age is a household decision, not an individual one

Most claiming calculators optimise one person's breakeven. The larger number is usually the survivor benefit: when the higher earner delays, they are raising the check their spouse may live on for decades after the first death. For a couple with meaningfully different earnings records and health expectancies, that single variable can outweigh the entire breakeven analysis.

3. Long-term care is a portfolio risk with no ticker

about 70% of people turning 65 will need some form of long-term care (ACL.gov), and Medicare pays for almost none of it. An unfunded care event does not just cost money — it draws down the exact assets the plan was using to fund the healthy spouse, at the exact moment the household's income need has gone up. It is the sequence-of-returns problem with a human being attached.

Why these three, together

They are not three errands. They are one plan with three moving parts, and the parts move each other.

  • A Roth conversion or a business exit raises MAGI, which raises Medicare premiums two years later. Covered in detail on how IRMAA blindsides HNW clients.
  • Claiming early while still working triggers the earnings test, and the same wages feed the IRMAA lookback.
  • Funding long-term care is a reallocation decision that touches the portfolio you manage — which is exactly why it should not be made by someone who cannot see it.

An advisor who hands each piece to a different vendor gets three correct answers that do not add up. That is the argument for one specialist across all three rather than a Medicare agent, a claiming tool and an LTC brochure.

What you actually get

Credentials, not enthusiasm

CLTC for extended care and RSSA® for claiming analysis, plus Medicare appointments and annual certification. Licensed in 30 states.

A written recommendation

You get a summary of what was recommended and why, in language you can put in a client file. Not a phone call and a hope.

An annual re-check

Formularies and networks change every plan year. A plan that fit in 2022 often does not in 2026, and nobody re-shops a client who is not being watched.

The independence question

Worth asking of anyone you refer to, including us. SkyWell is independent and represents multiple carriers rather than being owned by one, so there is no single-carrier quota shaping a recommendation. We also state plainly, on every page of this site, that we do not offer every plan available in a given area — because no agency does, and one claiming otherwise is telling you something about themselves.

See how a referral actually runs.

The framework page walks through it step by step, from intake to the annual review.

Frequently asked questions

Can't I just refer to a general insurance agent?

You can, and for a simple Medicare Supplement it may be enough. The three risks here interact — claiming age drives taxable income, taxable income drives IRMAA, and an unfunded care event drains the same assets your plan is projecting. An agent who handles one line at a time will not see those connections, because they are not looking at the plan.

My broker-dealer restricts outside business activity. Does that block this?

Usually not, because in most arrangements you are making a referral rather than conducting insurance business. What your firm needs to approve varies, and any revenue-share arrangement definitely needs their sign-off. We would rather build it the way your compliance department wants it than find out afterwards.

What if my client already has Medicare sorted?

Then the useful question is when it was last reviewed. Plans change their formularies and networks every plan year, and a plan that fit in 2022 often does not in 2026. A no-obligation review costs the client nothing and frequently finds something.

How much of my time does this take?

An introduction and a short handoff note. We run intake, analysis and enrollment, and send you a written summary of what was recommended and why. You do not attend meetings unless you want to, and plenty of advisors do want to on the first few.
